Circle the City creates and delivers integrated healthcare solutions that compassionately address the needs of all individuals facing
homelessness.

We do this through five lines of service:

Two fixed-site family health centers offering primary and preventive care:

Two medical respite centers:

  • Each medical respite center offers 50 beds for those who have been living
    on the streets with acute medical conditions and for people facing
    homelessness being discharged from valley hospitals who need additional
    time to complete their recovery.

Five mobile medical clinics:

  • Our mobile medical teams provide primary health care services to patients across Maricopa County who cannot or will not access care at a fixed site.

Street Medicine:

  • Our street medicine teams provide backpack medicine and mental health services to people facing homelessness in encampments throughout Maricopa County.
  • Service locations may vary, call 602-516-3072 for location info.

Health Navigation:

  • Our health navigators work together with hospital staff to help connect patients facing homelessness to support services upon discharge.

What to expect at an entry point:

  1.  Interview: Information about your situation will be gathered so that you are connected with appropriate services.
  2. Resource Connection: Based on your specific situation, you will be provided with resources that fit your need.
  3. Housing Plan: Shelter options and a housing plan will be discussed

Key items needed for housing:

  • Proof of Income/Job History
  • 2 valid forms of identification (state ID, Birth Certificate, Social Security Card)
  • Government Benefits such as SNAP, AHCCCS and/or Medicare
  • Phone number where you can be reached
