What We Offer
Circle the City has two outpatient clinics that provide primary and preventive healthcare. We support both walk-ins and scheduled appointments.
Outpatient care, also known as ambulatory care, is defined as a service or treatment that does not need to be conducted in a hospital setting or does not require an overnight stay following a procedure.
Services Include
- Medical evaluation of illness or injury
- Preventive care
- Evaluation and management of chronic diseases
- Immunizations, including COVID-19 and seasonal flu vaccine
- Behavioral health and substance use consultation
- Case management including assistance with IDs, social services, and housing
- Transportation assistance
- Care coordination
- Pediatric care including well-child visits at Parsons Family Health Center
