What We Provide
Circle the City’s Street Medicine Outreach engages unhoused individuals who do not readily access primary healthcare services in the community by delivering this care in river bottoms, canals, alleys, street corners, parks and other areas. Street medicine care is transitional by connecting patients to comprehensive medical and behavioral health services.
A critical component of the outreach model of care delivery is meeting the individual where they are physically and emotionally. The street outreach approach to care is predicated on the foundational idea that bringing services directly to the patient is the first step toward developing and maintaining a therapeutic relationship.
Street medicine outreach fills a gap in essential safety net services and is often the only medical care that many people facing homelessness are able or willing to access.
